Important Information:
This form is for logging usage of Traffic 1 and Traffic 2 vehicles. FUEL the vehicles after use to full if less then 3/4 tank. If unable to fuel contact Jeff Smith (9808) at 574-220-1640 or Blain Pearl (9826) via phone call/ text and leave/send a message advising truck name, fuel level and that you were unable to fuel the vehicle. REMEMBER the vehicle must be fueled to at least 3/4 full when returned. If there is any MAJOR mechanical issues immediately contact Jeff (9808) at 574-220-1640. If he does not answer leave a message/text and try to contact Blain (9826) and do the same.
